      The Office of Aging and Adult Services (OAAS) was formed within the Louisiana Department of Health (LDH) as a healthcare reform initiative, bringing together all of the long-term care programs that serve senior citizens and people with adult-onset disabilities.

      The Program Monitor/My Place Transition Coordinator functions in support of Research and Quality Division and is responsible for the coordination, facilitation, and monitoring of all demonstration participant transitions within domiciled territory as outlined in the state MFP Operational Protocol. As needed, responsibility extends to directly assisting OAAS regional office staff, enrolled support coordination agencies, and other waiver providers as requested for participant’s transition needs. Other responsibilities will include participating in and providing trainings to all providers, disseminating informative materials regarding OAAS services and supports in regards to HCBS, and facilitating resolution of individual transition issues at the agency programmatic level.


      Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

      Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

      A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

      An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

      EXPERIENCE SUBSTITUTION:
      Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

      • Transition coordination and support. Facilitate and monitor all transitions with the assigned geographic location of the state.

      • Implement operational protocol in OAAS program areas, as well as all public and private state-operated licensed nursing facilities.

      • Program integrity. Monitor the status of the program portion focused on older adults and persons with adult-onset disabilities against individual data indicators and transition benchmark targets.

      Preferred experience:

      • Excellent communication skills, both written and verbal
      • Ability to think critically and creatively solve problems
      • Experience managing multiple assignments to completion
      • Skilled in meeting deadlines with the ability to manage time effectively
