      Job summary

      The hours are part-time, 30 hours per week.

      This vacancy is for the Oakwood Health Visiting Team in Cheltenham.

      The specific job purpose is to support the Public Health Nursing Team 0-19 years. In the delivery of services to ensure the health needs of the population are met, through developing and maintaining close partnership working with multi-agency teams such Early Years, SEND and Social Care.

      The role of the Public Health Nursing Service is to deliver government programmes to improve the lives of children and their families. The Healthy Child Programme (DH 2009) is a cross-government strategy that is currently used to deliver a core universal programme, which is led by Specialist Community Public Health Nurses. The Public Health Nurse is a Practitioner working with children and families within the community, working in an effective, safe and inter-professional manner with all agencies.

      Main duties of the job

      The role includes supporting families and children with special educational needs and disabilities (SEND) and support their progression on the Gloucestershire Health and Care NHS Foundation Trust (GHC) policies such as the SEND Graduated Pathway for Gloucestershire, SCAAS pathway, Safeguarding, promotion of health, prevention of ill health & protection of children are key priorities. This is undertaken in partnership with other agencies.

      The Public Health Nurse works under the instruction and supervision of a SCPHN-Health Visitor in accordance with GHC protocols and competencies.

      • Responsible and accountable for assessing and implementing programmes of care from the Healthy Child Programme, in collaboration with the Specialist Community Public Health Nurse.
      • This may also involve other evidenced based programmes ofcare in partnership with clients or other agencies to identify and address health needs.
      • Progress the level of intervention as may be required according to identified risk factors which potentially could impact on a child's health outcomes.
      • Provide leadership and act as a role model by representing the values and beliefs of the nursing profession in addition to aligning to the GHC Values Framework.

Our programme is provided to all Newly Qualified; Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module Preceptorship for Healthcare (AP7026).

      Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.
