      We're looking for someone to join our team as a Quality Care Coordinator who:

      The Quality Care Coordinator will report to the Quality Care Coordinator Supervisor. Quality Care Coordination, at Clinica Sierra Vista (CSV), allows primary care physicians to use the dedicated time to direct proactive care for their patients, uses staff support to conduct outreach, and leverages new panel-based information technology tools.

      Essential Functions:


      • Assist with the daily operations and support within the Quality Improvement department including data management, and data reporting needs.
      • Responsible for medical record retrieval projects related to HEDIS.
      • Outreach to patients (new and existing) via telephone, text, and patient portal to assure care needs are being met and all care gaps are addressed.
      • Meets clinical quality measure indicators.
      • Provides qualified data to Supervisor to incorporate into training programs, policies, and procedures.
      • Recommend interventions to Supervisor so the practice may improve clinical quality and operational efficiency.
      • Collaborate with health centers as assigned, to facilitate redesign efforts, support care teams, introduce population health management, and support achievement towards improved financial, process, and clinical outcomes.
      • Support the data management and quality improvement initiatives.
      • Ensure data review process for assuring accuracy and integrity of health services data reports.
      • Report outcomes and quality monitoring results to the supervisor.

      You'll be successful with the following qualifications:

      • Medical Assistant certification or program completion required. Existing employees in this role prior to April 25, 2022 will be grandfathered in.
      • High School Diploma or GED required.
      • Minimum of 3-4 years of healthcare experience. Preferably in the ambulatory care setting. Working knowledge of HEDIS, UDS, PCMH
      • Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.
