      A Brief Overview

      Ensures patient receives post-imaging care, as recommended in the incidental finding notification, from high value network providers and follow up appointments are scheduled. The Radiology Central Navigator (RCN) plays an essential role in the patient's on-going treatment plan. The RCN is responsible for closely monitoring and tracking incidental finding alerts to ensure patients receive timely care in response to radiology findings.

      What You Will Do
      • Responsible for maintaining patient confidentiality while working in hospital systems.Maintains up-to-date knowledge of Radiology computer systems and departmental policies/procedures.
      • Within 72 hours of incidental alert
        • Receive and review alert notifications.
        • Initiate care plan based on the status of recommendations.
      • Provider Relations
        • Establish and maintain strong business relationships with UH Clinicians and staff on UH services.
        • Monitor, track, and close incidental finding alerts.
        • Route alert notifications accordingly.
      • Connecting with patients
        • Coordinate appointments for diagnostic testing and UH specialists/programs in an appropriate time frame and optimal location.
        • Answer general questions about UH services.
      • Archives alert once all needed actions are complete.
      Additional Responsibilities
      • Performs other duties as assigned.
      • Complies with all policies and standards.
      • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
      • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
      Education
      • High School Equivalent / GED (Required)
      • Associate's Degree (Preferred)
      Work Experience
      • 5+ years Customer Service Relations (Required)
      • 2+ years Experience facilitating telephonic outreach (Required)
      • 2+ years Experience working in healthcare setting (Preferred)
      • 1+ years Experience using clinical computer systems (Preferred)
      Knowledge, Skills, & Abilities
      • Must have high integrity level and ability to maintain confidentiality of sensitive data (Required proficiency)
      • Excellent verbal and written communication skill (Required proficiency)
      • Time management and organizational skills (Required proficiency)
      • Excellence in customer service with all stakeholders (patient, employer staff, providers, etc (Required proficiency)
      • Ability to work independently with minimal oversight, maintaining and demonstrating productivity and professionalism (Required proficiency)
