      We are looking for a Community Support Worker to join the South Hams Core Adult Mental Health Team based at Leatside Surgery, Totnes.

      The role is dynamic and focused on rehabilitation and recovery. The role will provide direct therapeutic engagement to services users focus on desired future goals and solution building.

      We will support you to learn the skills you need to thrive in the role. We will offer you the training and development support you need to begin a fulfilling career. In return, you will need to be motivated, flexible and eager to learn.

      Main duties of the job

      The support worker will have experience in supporting people with mental health difficulties.

      The support worker will be able to support service users with emotional, social, physical and domestic needs. The support worker will be skilled in engaging both service users and stakeholders and have a good understanding of the needs and diversity of those you will be supporting.

      The support worker will assist clients with their activities of daily living.

      The Support Worker will develop strong links with carers and other networks to support the service user to develop a co-produced management plan that allows them to manage their mental health and build on existing skills and community links to further support their future management and wellbeing.

      The team are currently developing physical health monitoring clinics, the support worker will have an instrumental role in supporting the running and development of these.
