Main duties of the job
The service will enable people to receive an intensive community rehabilitation service closer to home in their own community. We are transforming with the NHS Long Term Plan (2019) for Mental Health at the heart of our innovations along with North Lincolnshire Health and Care Partnership Community First Strategy.
The community rehabilitation service will consist of a multi-disciplinary, multi-agency workforce, working collaboratively between Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) and NorthLincolnshire Council to share skills and specialisms to support individuals and improve their quality of life.
The team will be led by a team manager and will have dedicated admin support.
