      Overview:

      Provides support, resource identification, and education to forensic outpatient clients with an emphasis on re-entry needs including but not limited to aftercare services, employment, benefits, and housing. Assures intra-agency and community continuity of care for assigned caseload. Prepares and maintains client records in accordance with all applicable regulations and standards.

      ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES

      1. Develops and updates individualized Client Service Plans (CSPs) for assigned cases in accordance with Client Record Handbook. Meets requirements of 90% of CSP/SPR completed.
      2. Meets Service Productivity standard of at least 62.5% (or 5 out of 8 work hours) and completes Progress Notes for all service activities in accordance with Client Record Handbook specifications and Agency business/clinical ethics standards.
      3. Provide outreach and support services to high-risk populations involved with the criminal justice system, including those involved with local law enforcement, those served by the courts, and those returning from state mental health facilities. This includes time limited case management services to connect clients to the appropriate clinical program for follow-up services upon case resolution.

      Qualifications


      Education/Experience:

      • Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, health science, or a related human services field;
      • One year experience working with individuals with severe and persistent mental illness preferred.
      • Or other Bachelor's degree from an accredited university or college with three years full-time or equivalent experience working with adults experiencing serious mental illness.
      • Professional work experience in a forensic setting preferred.

      Licensure or Certification:

      • Must maintain current Targeted Case Manager Certification.
      • Must possess current, valid driver's license and maintain Apalachee Designated Driver status.
