SUMMARY:
The Internal Referral and Retention Coordinator plays a crucial role in ensuring that patients within Gosnold Behavioral Health’s continuum of care remain engaged in services. This individual works closely with case managers to retain patients, actively participates in patient rounds to review discharge plans, and intervenes when external referrals are unnecessary. When an external referral is required, the Internal Referral and Retention Coordinator ensures the patient returns to Gosnold’s continuum after discharge. This role focuses on protecting and optimizing our internal care continuum.
ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:
Collaborate with case managers to retain patients within Gosnold’s continuum of care.
Attend patient rounds to review all discharge plans and intervene when patients are being referred externally when internal options are available.
Develop and implement strategies to maximize patient retention within Gosnold’s programs.
Build and maintain relationships with external providers to ensure patients referred out return to Gosnold’s continuum upon discharge.
Monitor and track patient transitions to ensure continuity of care.
Work closely with clinical teams to identify potential gaps in Gosnold’s continuum and provide feedback to leadership.
Ensure smooth communication between internal teams and external providers regarding patient transitions.
Provide reporting and analysis on patient retention metrics and referral patterns to leadership.
Collaborate with internal teams, including admissions, clinical staff, and administrative personnel, to support patient retention efforts.
Act as a liaison between Gosnold and external providers to facilitate seamless patient transitions back into our programs.
Participate in meetings and initiatives aimed at strengthening Gosnold’s internal referral and retention processes.
Certificates, Licenses, and Registrations: Valid MA driver’s license required.
MINIMUM KNOWLEDGE, EXPERIENCE, AND SKILLS REQUIRED:
The requirements listed below are representative of the knowledge, skills, and/or abilities required to perform the job.
Education: High School Diploma. A degree in Business Administration, Management, Accounting or Health Care Administration is preferred.
Experience: Minimum of one (1) year of experience in healthcare business development, patient retention, or case management. Understanding of behavioral health services and market dynamics.
Skills: Strong knowledge of behavioral health services and the importance of care continuity. Excellent communication and relationship-building skills. Ability to collaborate effectively with clinical and administrative teams. Strong organizational and problem-solving skills. Ability to track and analyze referral and retention data. Commitment to Gosnold’s mission and patient-centered approach.
