      An exciting opportunity to be part of the new structure at HMP Wormwood Scrubs. The referral coordinator will work within the early days team in ensuring that new prisoners receive the right care at the right time. You will be responsible for managing all referrals for men within the first 14 days of custody. You will be involved in the daily referral meeting and work within a multi-disciplinary team to ensure that patients are sign posted to the relevant clinical teams. You will be using Systm One and be expected to navigate through a high volume of referrals. You will work collaboratively with a wide variety of clinical and non-clinical team members across the partnerships.

      Main duties of the job

      The successful candidate will have a wide range of experience of office routines and procedure, working with multi-disciplinary teams in a customer care environment.

      Youll receive an annual salary up to 27,300 per annum - dependent on experience.
