  Schedule: Combination of weekday and weekend 12 hour shifts

  Trillium Gift of Life Network (TGLN), part of Ontario Health, is responsible for the coordination of organ and tissue donation and transplantation in Ontario. Our mandate includes increasing public awareness of the importance of organ and tissue donation, education of health care professionals, and ensuring a safe and effective donation and transplant system in Ontario.   We are currently seeking a part-time 0.75FTE Referral Triage Coordinator (RTC) for an 12-month contract position. The RTC is responsible for triaging every mechanically ventilated referral received from hospital front line staff to determine preliminary suitability for organ donation. The RTC collaborates with hospital front line staff in developing plans to ensure opportunities for organ donation at end of life are preserved.

  The RTC may also manage out of Province organ offers: collect donor information, test results and relevant organ specific information required for organ allocation/offering, make arrangements for additional testing and organ recovery as required.

  The RTC may also approach for consent via telephone if/when an Organ and Tissue Donation Specialist/Family Discussion Coordinator is not available or when timing is an issue.

  Here is what you will be doing:

  • Provides timely, consistent support to hospitals and acts as an expert resource for organ and tissue donation processes and practices; is able to influence and educate on-site (i.e. hospital) staff

  • Conducts chart reviews with front line staff from critical care areas (via telephone) to assess the potential for organ and tissue donation and completes documentation

  • Provides direction and support to health care professionals to preserve the opportunity for organ and tissue donation

  • Maintains records for: lab values, medications, diagnostic tests, and all other information required to evaluate and manage potential/actual organ and tissue donors

  • Manages new/open referrals and regularly updates status of cases to TGLN’s Management Team during daily rounds (bullet rounds); prioritizes multiple referrals to ensure donation potential is preserved

  • Determines, having processed the medical information, whether to refer a case such that resources will then be allocated; prioritizes cases to determine where to best deploy resources given limited resources and number of referrals

  • Collaborates with transplant programs, donor hospitals and other TGLN staff to assess suitability and optimize donation outcome; implements plans and gives direction in donor management to preserve and maximize opportunity for organ donation

  • Works closely with the health care team and other TGLN staff (Organ and Tissue Donation Specialists) to medically manage the patient’s physiological status in a manner consistent with established guidelines while working to sustain optimal organ function; the RTC may be called upon to act as a remote Organ and Tissue Donation Specialist in the event that there is no Organ and Tissue Donation Specialist present on site

  • Triages organ referrals and performs preliminary screening working with HCP’s and the referring OPO to obtain relevant clinical information

  • Coordinates laboratory services to facilitate STAT testing of blood samples; may arrange for transportation of samples

  • Interprets and communicates HLA, serology, and other laboratory information to transplant programs in order to assess donor and organ suitability

  • Generates organ allocation reports and applies organ allocation rules/policies to ensure that organs are offered according to established organ allocation algorithms

  • Works with transportation providers to facilitate logistics of organ recovery and arranges transportation of multiple recovery teams (surgeons, land and air transportation)

  • Maintains frequent communication with other OPO staff to ensure organ suitability is not compromised due to donor instability or delays in recovery or transportation

  • Maintains frequent communication with accepting transplant team members to ensure optimal recipient outcomes

  • Verifies information and consolidates documentation; Ensures all case related ‘touch points’ are documented and completed as per Health Canada requirements

  • Provides detailed summaries of cases that do not meet best practice guidelines to the Hospital Development Department to ensure follow up and facilitate quality improvement

  • Reports any errors and/or incidents to the Quality Department for follow up with Health Canada

  • Works with health care teams (by phone) to request information to assess tissue suitability for transplant or research and teaching

  • Responds in a timely manner to inquiries about tissue donation and/or seeks next of kin for follow up with regards to tissue donation

  • Documents and inputs donor information to TGLN databases; works with tissue and other TGLN staff to arrange recovery of consented tissues

  • Performs other duties consistent with the role, as required
    

  Here is what you will need to be successful:
  

  Education and Experience

  • Nursing Diploma and/or Bachelor of Science in Nursing is required

  • Current RN registration with the College of Nurses of Ontario

  • Minimum four (4) years critical care nursing knowledge and experience, and/or organ and tissue donation/transplant nursing experience is preferred
    

  Knowledge and Skills

  • Demonstrates strong understanding of applied pathophysiology, medical diagnostic testing/imaging, and ability to interpret laboratory results and vital signs is required

  • Proven ability to sort through large amounts of clinical information, understand/interpret the information and re-communicate the information to internal and external stakeholders

  • Proven ability to effectively relate to front-line health care teams and influence remotely to ensure donation is factored into plans for end-of-life care

  • Demonstrates conflict management skills and an ability to influence through applying expertise/rational arguments

  • Demonstrates experience in critical thinking, multitasking and prioritizing caseloads

  • Proven ability to work in a fast paced, time sensitive environment

  • Proven ability to problem solves in high pressure, time sensitive situations
    

  Rate of Pay: $33.65 – 48.17 per hour

  Employment Type: Part-time 0.75FTE Contract (12 months)
