What to Expect (Job Responsibilities):
- Participate in ongoing assessment, care plans, implementation, monitoring, and evaluation of outcomes
- Serve as a liaison between members, providers, and internal/external customers
- Oversee cases and referrals to ensure effective implementation of interventions
- Conduct Preventative, Therapeutic, and General Nutrition consultations digitally or telephonically
- Coordinate meeting the nutritional needs of the targeted population

What is Required (Qualifications):
- Bachelor's or Master's degree in nutrition and dietetics
- Completion of an accredited dietetic internship program
- 3 years of experience in a hospital, clinic, or home-based clinical nutrition setting
- Certified Case Manager (CCM) credential or obtain certification within 2 years of hire
- RD/RDN (Registered Dietitian/Registered Dietitian Nutritionist) by the Commission for Dietetic Registration, and Tennessee state LDN (Licensed Dietitian Nutritionist) License
