      Overview

      The Registered Dietitian (RD) will conduct nutrition assessments and follow-ups as needed to assess participants’ nutritional status and provide appropriate medial nutrition therapy (MNT), such as therapeutic and mechanically altered diets, oral nutritional supplements or enteral nutrition recommendations, dietary counseling, nutrition education, food security resources, and coordination of care with other IDT disciplines. The RD will support Child and Adult Care Food Program (CACFP) compliance by planning menus to meet requirements, conducting quarterly CACFP foodservice audits, and maintaining required CACFP documentation. The RD will support meal service by planning therapeutic menus, maintaining diet records, coordinating diet changes with the Food Preparation Coordinator, and conducting monthly food safety audits. The RD will conduct in-services as needed for the kitchen staff and/or IDT members to ensure accurate and evidence-based nutrition care practices.

      Corporate Setting:

      This role will support the Nutrition & Dietetics Manager directly over four major domains of work: staff coverage, training, administration, and project coordination. The RD will cover various PACE Registered Dietitian duties remotely or in-person for any of the AltaMed PACE centers, as needed to support compliance-related needs such as assessments, care planning, IDT meetings, and food program requirements. Will support training such as onboarding/orientation, process implementation, and competency re-training as assigned by the Nutrition & Dietetics Manager. This role will support Nutrition & Dietetics operations by assisting processes such as data collection, data analysis, report building, preparing and facilitating presentations, and compliance recordkeeping. In addition will support PACE by assisting with program planning and implementation, participating in CQI initiatives, and serving on interdisciplinary committees to improve participant health outcomes and the delivery of nutrition care services. The role will also collaborate with the Clinical Operations Team, QI, the Nutrition & Dietetics department, and other PACE leaders to address organizational goals and optimize care delivery.

      Responsibilities

    • Conducts nutrition assessments by conducting participant interviews and analyzing anthropometric data, biochemical data, pertinent medications, documentation from other providers, and other pertinent information to provide appropriate nutrition care — obtaining Primary Care Providers’ orders as required.
    • Assesses participant’s dietary patterns, food preferences, methods of preparation, and cultural and environmental factors affecting the participant’s dietary intake.
    • Conducts follow-ups with participants for focused assessments, nutrition education/counseling, and care coordination as needed and as determined by the IDT.
    • Educates the participant’s family or other caregivers about the need for and preparation of therapeutic diets as part of the treatment plan.
    • Actively participates in IDT meetings to collaborate on participant care and develop plans of care, integrating the nutrition treatment plan into the overall plan of care developed by the IDT.
    • Re-evaluates and revises nutritional plans as necessary to ensure the continued adequacy and appropriateness of the nutritional plan — obtaining Primary Care Providers’ orders as required.
    • Completes clinical documentation, referral submissions, diet and oral nutritional supplement records, and CACFP food program records in a timely manner, maintaining records as required.
    • Reviews site-level menus to ensure CACFP compliance, modifying as needed to meet compliance and create therapeutic menus.
    • Provides technical assistance to the kitchen regarding proper provision of meals to meet participants’ medical needs and preferences, proper sanitation and safety procedures, CACFP requirements, or other topics as needed.
    • Conducts monthly sanitation and safety audits for compliance with DPH/CMS/TJC requirements, as well as quarterly foodservice audits for CACFP/USDA requirements. Collaborates with kitchen staff and kitchen supervisor to maintain requirements.
    • Corporate Setting

      Responsible for Job Functions 1-10 when covering PACE sites. Also responsible for the following Job Functions:

    • Conduct any of the PACE center RD duties (listed above) as needed to cover RD staffing needs, ensuring timely and accurate documentation.
    • Monitor and validate clinical nutrition and foodservice process and documentation requirements.
    • Assist with creation, review, and implementation of Nutrition & Dietetics department onboarding, training, and reference materials for Registered Dietitians and Food Preparation Coordinators.
    • Coordinate orientation of RD and Food Preparation Coordinator new hires in collaboration with Center Managers and the Nutrition & Dietetics Manager, ensuring training standardization across centers.
    • Will assist provision of targeted training for RDs and FPCs for new processes and in response to other identified needs.
    • Conduct staff meetings or trainings on clinical, foodservice, and compliance requirements.
    • Create trainings, guides, and user manuals to support daily use by department staff and interns.
    • Assist in assessing workflow, recordkeeping, technological, and material needs of the Nutrition & Dietetics department.
    • Organize and maintain compliance, personnel, and other departmental records.
    • Assist with planning and coordination of Dietetic Interns from affiliated RD and Dietetic Technician programs.
    • Keep up-to-date on changes in Nutrition & Dietetics and PACE requirements, rules, and regulations.
    • Assist with planning, implementation, and outcome evaluation of new programs and processes—coordinating and meeting with other disciplines and company divisions as needed.
    • Participate in state and federal audit preparation activities.
    • Perform all other duties as assigned.
    • Qualifications

    • Registered Dietitian or Registered Dietitian Nutritionist status with the Commission on Dietetic Registration required.
    • Prior experience in clinical nutrition as Registered Dietitian and experience working with the frail elderly preferred.
    • Current Valid BLS Certification
    • Bilingual English/Spanish/Cantonese/Mandarin preferred.
    • Food Protection Manager Certification preferred.
