Consultative Nurse (Intellectual Disabilities Services)
Registered Nurse 24 Hours Weekly (TEMP to PERM)
Days & Hours Flexible
Claymont, DE (Home visits mostly in New Castle)
$42 per Hour
Reports to the Executive Director
Education and Experience
• Bachelor’s Degree in Nursing or equivalent.
• Nursing or healthcare management experience.
Professional Credentials/Certifications
• Delaware Registered Nursing License in good standing.
Position Requirements
• Certification in First Aid and CPR
• Certified to teach Limited Lay Administration of Medications (LLAM) – Certification may be provided while employed.
• Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.Responsibilities
• Manages the facilitation of health care service needs of individuals served and ensures that these service needs are addressed.
• Complete, when necessary, all required medical assessments pursuant to best practices, contractual obligations, and licensure requirements.
• Ensures that an individual's health care services adhere to the best practice, licensure requirements, and are consistent with the policies, procedures, and protocols of contractors and payers of services.
• Oversees effective wellness and medical services in accordance with policies and procedures; state and federal regulations; and extant professional, ethical, and legal principles.
• Models a commitment to the mission of the organization
• Maintains an effective therapeutic alliance with individuals served.
• Directs medical care coordination activities and meetings.
• Completes all required documentation, audits, reviews, and reports in a timely manner relative current and ongoing health care service needs, including, Monthly Medication and Health Audit, the Nursing LSP Quarterly Report.
• Communicates and coordinates with individuals receiving services, healthcare providers, family members or legal guardian, and staff regarding individuals’ health care needs.
• Utilizes the required Electronic Health Record to record, document, and monitor all health care encounters and interactions.
• Abides by state and federal regulations, policies, and procedures related to confidentiality, patients’ rights (PM 46), and exposure and infection control.
• Provides Limited Lay Administration of Medications training to staff who assist with medications,
• Attends and participates in required meetings and trainings, including the monthly Nursing Best Practice meeting.
• Coordinates and provides after hours on-call support to program staff.
• Performs other duties as requested or assigned, verbally or in writing.

• Clinical Knowledge: Possesses nursing and medical assessment and intervention skills, specifically in utilizing evidence-based practices and implements these skills in a professional manner.

Demonstrate sound judgment by gathering and analyzing information skillfully and taking appropriate and timely actions regarding questionable findings or concerns.• Leadership: Facilitates organizational change as it relates to HR and the use of data; has the ability to understand, relate with, and adapt to different cultural settings; and demonstrates good judgement and maturity.
• Critical Reasoning: Excellent observation skills and the ability to combine pieces of information to form general rules or conclusions, as well as extracting themes or patterns from seemingly unrelated pieces of information
• Inductive Reasoning: Keenly observes and gets to know clients, anticipates how client may react.
• Written Communication: Ability to communicate effectively in written form including, editing work for spelling and grammar, present numerical data clearly, concisely and accurately.
• Oral Communication: Ability to orally communicate with others, including providing direction and feedback, exchange of information and ideas, explain complex concepts and processes, utilize effective non-verbal skills, ask insightful questions, and persuade others.
• Time Management: Ensure that tasks/duties/activities, projects and goals are accomplished accurately and timely.
• Quality Control: Demonstrate accuracy and thoroughness with understanding of clinical documentation and the requirements for state standards and expectations. As well as details related to appointments, deadlines, goals, and client needs
• Adaptability: Ability to multitask, prioritize, and adapt to changes in the work environment; manage completing demands and ability to deal with frequent change, delays or unexpected events; and ability to work under emergency conditions, when applicable.
• Dependability: Ability to consistently report to work and on time, follow instructions, respond to management direction, and solicit feedback to improve performance.
