      The Registered Nurse Assessment Coordinator (RNAC) provides for the initial assessment and periodic re-evaluation of all residents admitted to Skilled Nursing. Conduct Interdisciplinary Team Care Conference and implement and update resident Plan of Care.

      The Registered Nurse Assessment Coordinator (RNAC) would:

      • Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care (CMI).
      • Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care.
      • Nursing Educator: – Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough. This requires high levels of in-person interfacing with both residents and employees.
      • Nurse Management: Serves as a key member of the nursing leadership team to clinically manage the unit in order to maximize patient-centered outcomes. This includes engagement in leadership standups and influence within interdisciplinary teams.
      • Case management w/ managed care (maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes)
      • Manage clinical and financial record keeping and dissemination of these records to appropriate payers. This includes overseeing UMR reviews and any appeals processes.
      • Take joint ownership of care conferences with social workers
      • Perform other duties as required.

      Qualifications

      Related Experience:

      • Minimum 2 years related experience; Knowledge of state and federal regulations and requirements under Omnibus Budget Reconciliation Act (OBRA).

      Education or Degree Requirement:

      • Graduate of accredited School of Nursing; Current license with the State of Pennsylvania for Registered Nurse

      Licenses, Certifications, Registrations:

      • Registered Nurse – licensed in State of Pennsylvania not required but highly desired; AANAC certification (desired, must have within 12 months of employment);Must maintain current CPR Certification.
