- Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA).
- Demonstrated recent experience as a Registered Nurse in an environment appropriate to the relevant speciality.
- Demonstrated ability to provide safe compassionate quality care that is culturally responsive.
- Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
- Demonstrated capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise.
- Demonstrated evidence of the use of critical thinking as it relates to the delivery of evidenced based nursing care.
- Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace.

The Transitional Aged Care Program (TACP) is seeking a hardworking, enthusiastic Registered Nurses for their community team in Bowral.
As a TACP Registered Nurse you will be responsible for providing community based nursing care to clients in their homes. Your clinical caseload will include providing appropriate assessment, education, intervention and discharge planning to clients currently accessing TACP.
You will enjoy seeing people in their own homes, working both independently and as part of a multidisciplinary team. You will have strong clinical judgement and problem solving skills.
This is an exciting opportunity to be part of a dynamic and energetic multidisciplinary Transitional Aged Care Program team within the SWSLHD, with opportunities available across our district.
TACP is a unique program focused on the re-enablement of older adults living in the community and the TACP Registered Nurse will play a pivotal role in ensuring the wellbeing and health of these clients.
Working with clients, their carer’s, the TACP team and other relevant stakeholders, you will be contributing to holistic care that ensures better health outcomes and quality of life over a time-limited period (up to 12 weeks) post-discharge from hospital.
We are currently seeking to fill permanent part time or full time role for our Bowral TACP team.
