      About the Role
      • The Dementia Advisory Service (DAS) RN works collaboratively with the DAS CNC1 – the role is 3 days a fortnight with a contract end date of 30/6/2025.
      • The RN position is a supportive role to the CNC1 and a willingness to learn and be mentored is crucial.
      • Working with the CNC1, the RN will be providing client-centred care and support to carers of clients living with dementia.
      • Duties will include, but not limited to: accepting referrals, triage and assessment of referrals, phone support to carers, attending home visits to complete comprehensive assessments, assisting with Carer Support Groups, education to Health and non-Health organisations.

      Where you'll be working
      • Based at Muswellbrook Community Health Centre, you will be working within the Hunter Valley Community Health team.
      • The Dementia Advisory Service covers a large geographical area that includes: Muswellbrook, Scone, Murrurundi, Denman, Merriwa and Cassilis towns and surrounding areas – within the LGA’s Muswellbrook and Upper Hunter Councils.
      • The HVCH Nursing team comprises multiple disciplines: Community Nursing, Child and Family Health, Palliative Care, Women’s Health and Families NSW Perinatal Service.
