- Conduct initial and ongoing client nursing assessments in the home setting.
- Develop individualized plans of care in accordance with client needs, physician orders, and regulatory guidelines.
- Perform supervisory visits in compliance with SC DHEC Section 6 regulations, including:
- Every 62 days or more frequently as needed for aides.
- Within 30 days of the start of care for new clients receiving personal care.
- Monitor and evaluate the performance of home care aides and provide training and correction as needed.
- Document all nursing services, supervisory visits, and client interactions in compliance with state and agency requirements.
- Communicate with clients, families, caregivers, physicians, and other healthcare professionals to coordinate and optimize care.
- Maintain accurate, up-to-date clinical records and participate in quality improvement initiatives.
- Respond to changes in client condition and coordinate appropriate follow-up care or interventions.
- Ensure all care is delivered according to agency policy and state and federal regulations.
- Current South Carolina RN license in good standing.
- Minimum of 1 year of nursing experience, preferably in home care or community health.
- Working knowledge of South Carolina DHEC Section 6 home care regulations.
- Experience conducting supervisory visits and training aides is strongly preferred.
- Strong interpersonal, organizational, and communication skills.
- Reliable transportation and a valid driver’s license.
- Field-based work in client homes across Hilton Head and surrounding areas.
- Flexible scheduling with some evening or weekend visits as needed.
- May require occasional lifting or physical assistance during client care.

ComForCare Home Care Hilton Head is seeking a compassionate, detail-oriented, and licensed Registered Nurse (RN) to join our team. The RN will play a critical role in ensuring high-quality care for our clients in accordance with Section 6 of South Carolina Department of Health and Environmental Control (DHEC) home care licensing regulations. This includes conducting initial client assessments, developing care plans, and performing regular supervisory visits to oversee care provided by unlicensed personnel. This is a 1099 position.
