- SOC visits: $100 to $120
- Routine revisits: $65 to $75

- Conduct comprehensive assessments and develop personalized care plans
- Deliver skilled nursing services such as wound care, IV therapy, and medication management
- Educate patients and families on health conditions, medication routines, and home care best practices
- Maintain clear and timely documentation using EMR
- Collaborate with physicians, therapists, and other team members to coordinate care
- Support patient safety, comfort, and independence in the home setting
- Active and unrestricted Registered Nurse license in Michigan
- Minimum of 2 years of nursing experience, ideally in home health care
- Strong assessment skills and clinical judgment
- Proficient in wound care, IVs, and medication administration
- Compassionate, reliable, and detail-oriented
- Familiarity with EMR systems
- Valid driver’s license and reliable transportation for patient visits
