- Provide continuous assessment of patients and their carers, identifying evolving needs across a varied caseload.
- Deliver skilled nursing interventions including medication administration (via multiple routes), verification of death and assessments for care.
- Offer psychosocial support to patients and families experiencing emotional distress, providing compassionate, evidence-based guidance.
- Communicate effectively with multidisciplinary healthcare professionals, social workers, patients, carers, and managers across organisational boundaries.
- Facilitate formal educational training sessions for carers and community RNs.
- Contribute to clinical audit and research projects to drive continuous improvement and best practice.
- Be a Registered Nurse with valid NMC registration and at least 6 months post-qualification experience.
- Have proven experience in managing complex patient needs in a community setting.
- Strong clinical knowledge and the ability to adapt to rapidly changing situations.
- Excellent communication and collaboration skills across disciplines and roles.
- Have flexibility to work across day and weekend shifts.
- Full UK driving licence and access to a vehicle with business-use insurance (for community role).

We offer 24 hour / seven days a week palliative care to people who have advanced disease, caring for people in our Inpatient Unit which accommodates 15 patients and the local community. Our person centred care is delivered with compassion and respect for peoples wishes. We have an expanding Therapeutic & Wellbeing service offering Psychological Wellbeing, Social Care, Physiotherapy, Occupational and Complementary Therapy.
