Job summary
We have an exciting opportunity for a Registered Nurse in the Virtual Care Team.
We are seeking a dynamic, forward-thinking professional who wants to help us support patients in their own homes via the use of a remote monitoring platform. A commitment to team working and providing excellence in patient care is a must as are exceptional communication and people skills. An ability to prioritise and manage own daily workload is required in this role.
You will need to be able to demonstrate real enthusiasm for Virtual Care Nursing, supported self management and passion for innovation in Nursing.

This is a home based role that may requires travel for training and team meetings. Fixed term contract will end 24th July 2026.

Main duties of the job
Responsible for the assessment, planning, implementation, and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.
To work in partnership with patients and carers to offer support and advice and promote informed choice.
To support the role of the ward manager and band 6 staff.
To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures, and guidelines.
To support all locality teams by working in a flexible way.
