The Northport VA Medical Center is looking to hire two Registered Nurses for the Community Living Center. The Staff RN provides direct and indirect nursing care in a self-directed manner to Veterans/Residents and families, caring for Veterans/Residents requiring long term care, dementia care, palliative care, hospice care, rehabilitation, continuing care, restorative care, skilled nursing, respite, secure unit for behaviors, and geriatric evaluation and management.
Duties
The Registered Nurse (RN) is expected to communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers. The Staff RN must be able to push-pull and lift 35 pounds and must be able to stand for prolonged periods of time. Finally, the Staff RN is expected to work in a collaborative, team-oriented environment, and demonstrate consistent effective communication skills and exemplary professional behaviors. Fostering positive interdisciplinary collaborative relationships is essential for continuity of care that is safe and quality driven.
Nursing practice has an emphasis on health promotion, accountability, assessment, education, infection control, safety, and psychological support. A multidisciplinary approach is used to assist the patient, family and/or significant other(s) to attain optimal functioning ability. Measurable outcomes and nursing interventions are individualized.
The RN assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
The RN functions within a nursing process framework for problem-solving and care facilitation: assessment, planning, implementation, and evaluation. Job duties are focused on disease prevention and achieving optimum mental and physical health and well-being.
The RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require.
