      The Northport VA Medical Center is hiring a Part-Time Registered Nurse for the Community Living Center (CLC) evening and overnight shift.

      Duties


      The Staff RN provides direct and indirect nursing care in a self-directed manner to Veterans/residents and families in the Community Living Center), caring for Veterans/residents requiring long term care, dementia care, palliative care, hospice care, rehabilitation, continuing care, restorative care, skilled nursing, respite, secure unit for behaviors, and geriatric evaluation and management. S/he is accountable for adhering to established policies, standards of care and standards of practice related to the provision of care supportive to or restorative of life and well-being. The incumbent is actively involved in the residents' plan of care in maintaining or restoring their functional capacity by evaluating and/or recommending rehabilitative, therapeutic activities, and exercises based on the medical and social history of the residents. S/he assess, plans, implements, and evaluates care based on age-specific components, assuming responsibility for the coordination of care focused on resident transition through the continuum of care, Veteran/resident and family education, Veteran/resident self-management after discharge, and supporting factors that impact customer satisfaction. The incumbent reports directly to the Nurse Manager of the resident care unit/area and functions as team leader and charge nurse as assigned under the supervision of the Nurse Manager of designee.

      The Staff RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require. The Staff RN is expected to communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers. The Staff RN must be able to push-pull and lift 35 pounds and must be able to stand for prolonged periods of time. Finally, the Staff RN is expected to work in a collaborative, teamoriented environment, and demonstrate consistent effective communication skills and exemplary professional behaviors. Fostering positive interdisciplinary collaborative relationships is essential for continuity of care that is safe and quality driven.
