      We are looking for a motivated enthusiastic Registered Nurse with excellent communication skills to join the OPD at Chirk Community Hospital.

      The service is Mon-Fri and offers a wide variety of clinics, groups and activities for patients with lifestyle changes through disability or illness.

      The aim of the service is to provide supportive sessions for patients enabling them to live independently within their own home and community.

      Main duties of the job

      • Perform comprehensive assessment of patient nursing needs.
      • Keep up dated on new dressings/treatments
      • Be involved in trials and audit and monitor dressings and their effectiveness
      • Collate all information and document effectively.
      • Collect, collate and report information maintaining accurate patient records.
      • Recognise and respond appropriately to urgent and emergency situations.
      • Demonstrate proficiency in developing competence in clinical skills i.e. Venepuncture, ECG, Wound Assessments and effective management of skills
      • Involves patient/carer in planning and delivery of care needs.
      • Provide Holistic Palliative Care for patients with life limiting conditions keep up to date with policy changes and procedures.
