What to Expect (Job Responsibilities):
- Collaborate with a dedicated team to support a substantial patient panel alongside Primary Care Providers (PCPs)
- Triage inbound communications and escalate when necessary
- Act as a support system and escalation point for LVN/LPNs within the team
- Engage in comprehensive chart preparation and provide crucial patient education
- Oversee post-appointment processes and participate in daily huddles
What is Required (Qualifications):
- Current unrestricted RN license with at least 2 years of clinical experience
- Experience in remote roles and proficiency with technology
- Strong communication skills and ability to thrive in a fast-paced environment
- Experience with geriatric patients in senior living communities or skilled nursing facilities
- Familiarity with chronic care management and geriatric-related diseases and treatments
How to Stand Out (Preferred Qualifications):
- Experience in geriatrics or related fields
- Familiarity with healthcare technologies and digital communication platforms
