What to Expect (Job Responsibilities):
- Coordinate care for Veterans using health informatics and disease management principles
- Manage complex patient care in a cost-effective manner
- Utilize telehealth technologies to facilitate access to care
- Demonstrate leadership in executing care management strategies
- Ensure timely delivery of the right care to patients
What is Required (Qualifications):
- Registered Nurse (RN) licensure
- Experience in care coordination and case management
- Strong understanding of health informatics and telehealth technologies
- Proven ability to manage complex patient care
- Excellent communication and leadership skills
How to Stand Out (Preferred Qualifications):
- Experience in working with Veterans or in a telehealth environment
- Certification in case management or related field
- Familiarity with disease management programs
