Enters patient specific information into the technology platform at admission and deletes patient specific information on discharge.
Utilizes technology to communicate with patients, families, providers, and caregivers.
Follows appropriate patient privacy policies as a remote caregiver.
Completes annual attestation for remote role performance.
Complies with appropriate updating to equipment necessary to perform virtual nursing care.
Completes and documents accurate virtual based clinical assessments of patients on admission and onto admission forms throughout hospital stay according to established standards.
Ensures all discharge needs are addressed and discharged process requirements are complete. Coordinates with bedside Registered Nurse (RN) prior to discharge from inpatient practice area to home or with case manager to a facility.
Identifies actual and potential problems for which the patient is at risk. Demonstrates acquired knowledge and skills and a commitment to excellence at ChristianaCare.
Initiates, follows, and modifies a nursing plan of care based on assessment and diagnosis.
Assesses and documents patient response to interventions.
Utilizes the Teach-Back method while providing patient/family education based on learning needs and documents on appropriate software. Revises educational plans to meet needs as they change.
Encourages patient/family participation during virtual visits, in developing the plan of care, and in the discharge process.
Initiates and participates in team meetings and interdisciplinary discussions regarding patient care, including provider rounding. Elicits input from other team members. Hand-offs are accurate and include the patient or family’s input to assure concerns or needs are being addressed.
Performs Nursing Bundle components as able throughout patient encounter.
Demonstrates skills and knowledge necessary to provide care to neonatal, adolescent, adult, and/or geriatric practice areas including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs and to provide the care needed.
Establishes and maintains a sound knowledge base of nursing care necessary for specific patient population. Shares clinical knowledge through mentoring of others.
Utilizes effective verbal and written communication skills virtually with patients, families, caregivers, leadership, providers, and ancillary department staff. Demonstrates the ability to maintain healthy relationships with colleagues. Able to apply key Relationship Based Care skills in all interactions.
Effectively delegates duties and tasks to other nursing staff after assessment of capability and scope of responsibilities. Encourages each team member’s contributions.
Enhances the professional growth of self by staying current in clinical practice, incorporating research into practice and participating in councils, teams, committees, and task forces. Supports Nursing Professional Governance activities.
Represents the patient when the patient cannot represent self. Seeks available resources to help understand, formulate, and implement ethical decisions.
Participates in Nurse to Nurse Peer Feedback, self-appraisal, and personal developmental goal planning.
Performs assigned work safely, adhering to established ChristianaCare and departmental safety rules and practices; utilizes Report to Learn as necessary; reports to Nurse Manager in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other caregivers, patients and visitors.
Performs other related duties as required.
BSN Preferred
Commitment to obtaining BSN within three years of date of hire.
BLS required
2-3 years or more Registered Nursing experience
Certification in specialty preferred.
Delaware RN or ability to work in DE under current licensure.
