      MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

      Schedule: Monday–Friday, Day Shift (Full-Time, 40 Hours)

      Are you a Registered Nurse with a passion for quality improvement and making a statewide impact? We’re hiring a QI Field Services Manager to lead and support Quality Services across Massachusetts. This is your opportunity to apply your clinical background in a high-impact, leadership role with excellent work-life balance.

      In This Role, You Will:

      Quality Improvement

      • Lead quality assurance initiatives across programs.

      • Analyze data and identify opportunities for improvement.

      • Develop and implement state-specific continuous improvement plans.

      • Provide training, technical assistance, and process improvement strategies.

      Risk Management

      • Manage the incident reporting process and monthly trend reviews.

      • Support Operations with high-level internal investigations.

      • Reduce safety risks and enhance protocols across the network.

      Compliance

      • Oversee compliance with state/federal regulations and internal standards.

      • Conduct audits and support corrective action planning.

      • Guide policy development and ensure proper staff training.

      Customer Engagement

      • Lead efforts to improve satisfaction based on survey data.

      • Collaborate on local engagement strategies.

      • Coordinate the survey process in partnership with state leaders.

      What We’re Looking For:

      • RN or LPN license required

      • 3–5 years of experience with DPH regulations (Adult Day Health a plus)

      • Supervisory or leadership experience strongly preferred

      • Strong analytical, communication, and organizational skills

      • Comfortable managing projects, teams, and timelines across a broad geography

      • A proactive, collaborative, solutions-driven mindset
