      Job summary

      An exciting opportunity has arisen for a Registered Nurse to join our Willen at Home team.

      Main duties of the job

      The successful candidate will be a Registered Nurse, with a least 6 months post registration experience; you will have the responsibility for the assessment of patient needs, planning, delivering and evaluating care in the patients own environment. It's all about giving our patients the best possible quality of life, the support we offer is tailored to meet individuals' medical, social, spiritual and emotional needs.

      The ideal candidate should ideally demonstrate knowledge and experience working in the community and/or in palliative care and should participate in education and training activities.

      As the role is community based you should be confident working alone, must hold a driving licence and own your own car.

      Set in the tranquil grounds of Willen Lake, Willen Hospice is the leading provider of specialist palliative care within the Milton Keynes area. We offer 24 hour / seven days a week palliative care to people who have advanced disease, caring for people in our Inpatient Unit which accommodates 15 patients and the local community. Our person centred care is delivered with compassion and respect for peoples wishes.We have an expanding Therapeutic & Wellbeing service offering Psychological Wellbeing, Social Care, Physiotherapy, Occupational and Complementary Therapy.

      1. Continual assessment of patients and their carers needs, when those needs are complex and change across the caseload.

      2. Undertake nursing interventions (such as providing personal care to patients, administration of prescribed medication through a variety of routes, tissue viability care, ensuring infusion pumps are working correctly) consistent with evidence-based practice, transferring and applying knowledge and skills to meet individual patients needs.

      3. Provide psychosocial support to patients in emotional distress and their distressed families and provide a high standard of evidence based information to them.

      4. Communicating effectively with multi disciplinary health care professionals, medical and social work colleagues, patients, carers and managers across organisational boundaries.

      5. Manage the workload and liaise with M D T in the absence of the Lead Nurse.

      6. Participate in the delivery of formal educational training to carers and RNs in the community

      7. Participate in audit/research projects in order to achieve best practice.

      8. Participate in champion role work within the team to promote evidence-based practice and share learning to the wider team.
