      Job summary

      An exciting opportunity has arisen for us to recruit a Nursing Associate to join the new Community based Treatment Team within Community Mental Health across Bradford and Craven.

      As part of our commitment to improving patient care and outcomes, we are establishing a new treatment team specifically focused on supporting patients receiving depot medication. This is an exciting opportunity to be part of the development of a vital service that will have a lasting impact on the community.

      Main duties of the job

      Contribute to ongoing assessment, recognising when it is necessary to refer to others for reassessmentPerform nursing interventions advocated within the service user/patients care planInvolve service users/patient/carers in the planning and delivery of care in a compassionate and appropriate manner which considers their diverse needsMaintain the welfare and safety of service users/patients and protect them from any form of abuse in accordance with safeguarding guidanceMonitor the impact and effectiveness of nursing care deliveredThe successful candidate must be motivated, flexible, able to communicate effectively, will be expected to be able to travel independently to various locations across Bradford and Craven.Demonstrate a knowledge of information governance and the need to protect service user confidential informationMake and maintain accurate records of nursing care in accordance with the professional (NMC) guidance and Trust policyDemonstrate problem solving skills by identifying solutions in routine clinical issuesParticipate in regular clinical supervisionEngage in public health, community development and in the reduction of health inequalitie
