      Position Summary

      University Health Network (UHN) is looking for an experienced professional to fill the key role of Registered Practical Nurse at Hillcrest Reactivation Centre.

      Hillcrest Reactivation Centre is a 75 bed community based reactivation center, part of the short term transitional care sites within the TCLHIN. Operating under the Home and Community Care Act Hillcrest offers a short period of reactivation to clients who are ALC or at risk of ALC, supporting their return home.

      Duties

      • Plan, manage, and evaluate nursing care for clients and families within the RPN scope of practice
      • Use evidence-based best practices and collaborate with the inter-professional care team to plan and implement the best possible care for clients
      • Participate in inter-professional team care planning forums, inter-professional rounds and family conferences
      • Mentor, support and teach members of the healthcare team, including nursing students and new staff on the unit
      • Participate in professional development activities and program/hospital wide initiatives
      • Compliance with CNO standards and ethical nursing care for clients
