Job Description

- Work in collaboration with Public Health to provide health promotion and infection prevention and control education to a variety of stakeholders including but not limited to team members within long-term care homes, retirement homes, congregate settings, residents, families, and community partners.
- Perform onsite and virtual assessment of the various sites for Infection Prevention and Control (IPAC) related issues, and identified processes gaps, or need for training of others in the proper use of personal protective equipment (PPE), hand hygiene, environmental cleaning, and other IPAC-related topics.
- Provide written and verbal report on assessments and identified risks in congregate settings in a timely manner.
- Provide follow-up and ongoing IPAC support in consultation with Public Health.
- Frequent travel between congregate settings within Western York Region to provide assessments, staff education, audits etc., including enhanced on-site training for those facilities in COVID 19 outbreak.
- While on shift, responsible for responding to the community support hotline calls between 1600-1200 (midnight) 7 X week.
- Support on-site swabbing in congregate settings (group homes) as requested by York Public Health
- Willingness to follow Ministry of Long-Term Care directives including undergoing regular (daily) rapid swab testing prior to entering LTCHs.
- Registered Practical Nurse
- Current Certificate of Registration in the General Class with the College of Nurses of Ontario
- Certification in IPAC or IPAC extender training preferred.
- Must have a valid driver’s license and access to a vehicle.
- Current Basic Cardiac Life Support (BCLS) certification.
- Proven clinical experience with patient-focused care, including strong patient assessment, care planning, patient education and documentation skills.
- Must have physical strength, flexibility, and endurance required to perform the role based on assessment conducted by the Occupational Health and Safety Department.
- Experience working in Long Term Care Homes, Retirement Homes or Group Homes is an asset
- Knowledgeable of IPAC practices in congregate settings and ability to identify gaps and reinforce best practices.
- Exemplary interpersonal, communication, organization, and time management skills.
- Ability to build and maintain relationships with internal and external community partners.
- Flexible team player capable of working both independently and within a team environment.
- Demonstrate excellence in communication, both written and verbal.

Job Description
Reporting to the IPAC-Hub Manager, the Mackenzie Health IPAC-Extender is a member of Hospital IPAC team that will implement a robust plan to perform IPAC risk assessments, provide clinical recommendations, develop an education plan, support auditing, and provide training within congregate facilities for staff/families within the Western-York Region Ontario Health Team. The IPAC-Extender will serve as the bridge to the congregate facility to support and advise on the local implementation of public health best practices.
