What to Expect (Job Responsibilities):
- Engage with patients to navigate their healthcare, addressing questions and concerns
- Collaborate with medical practice staff and case management teams to manage patient populations
- Document patients' medical histories and issues accurately in electronic health records
- Implement and meet assigned Key Performance Indicators to enhance patient health outcomes
- Conduct compensated travel for patient and provider engagement within the Columbus, GA area
What is Required (Qualifications):
- Certified or Registered Medical Assistant OR Certified EMT with 2 or more years of experience
- Strong interpersonal skills with an emphasis on empathy and professionalism
- Experience with medical terminology and understanding of primary care physician practices
- Proficiency in Microsoft Office and familiarity with electronic health records
- Demonstrated ability to work successfully in a remote environment
How to Stand Out (Preferred Qualifications):
- Experience in patient coordination for individuals with complex medical and social needs
- Proficiency in motivational interviewing and behavioral change management techniques
- Knowledge of social determinants of health
