      What to Expect (Job Responsibilities):
      - Manage patient enrollment services for digital remote patient monitoring programs related to chronic diseases
      - Conduct patient outreach and education to ensure understanding of program participation
      - Coordinate and escalate care as needed, communicating effectively with providers and the healthcare team
      - Monitor alerts and engage with patients through digital platforms
      - Participate in Chronic Care Management initiatives, enhancing patient education and support

      What is Required (Qualifications):
      - Graduate of an accredited School of Nursing
      - Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Missouri
      - Strong verbal and written communication and facilitation skills
      - Strong assessment, time management, and critical thinking skills
      - Advanced computer skills and proficiency in digital tools

      How to Stand Out (Preferred Qualifications):
      - Experience in patient education or chronic disease management
      - Familiarity with digital health technologies and remote patient monitoring systems
      - Ability to work collaboratively in a multidisciplinary team environment
      - Additional certifications in nursing or healthcare-related fields
