Job Description
- Req#: 215015
- Participates in the design, conduct, and evaluation of implementation research and cohort studies focusing on transitional care interventions and their associated health outcomes
- Assists multidisciplinary hospital and ambulatory teams in the implementation of transitional care protocols and pathways
- Ensures patients and caregivers can utilize appropriate technology, such as electronic health record portals and video visits, as needed for intervention
- Follows an assigned cohort and outreaches patients telephonically to ensure effectiveness of interventions
- Collects qualitative and quantitative data from multiple sources including patients, caregivers, clinicians, and electronic health records
- Analyzes and interprets data in Excel, Power BI or other software
- Bachelor’s degree required, life sciences or public health background preferred
- MS Office proficient, Power BI or Tableau experience preferred
- Ability to manage multiple tasks and priorities with exceptional attention to detail
- Bilingual/Spanish-speaking preferred
- Evidence of excellent interpersonal and communication skills, creative problem solving, critical thinking and leadership
- Previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team
- Qualified candidates must be able to effectively communicate with all levels of the organization
Job Title: RESEARCH ASSOCIATE
The transitional care research associate is responsible for managing operational processes and supporting implementation and quality improvement research for the Transitional Care Excellence program. This position works with the transitional care excellence clinical team to ensure high quality and timely care in the hospital and post discharge.
