Job Description

- Document information pertaining to changes in residents’ status, needs and appointments.
- Ensure proper documentation of all care plans, nurses notes and all other resident correspondence.
- Strong experienced as a Medication Aide.
- Provide orientation and on the job training to Caregivers and Medication Aides.
- Assist with daily living activities (i.e., bathing, dressing, and eating).
- Measure, record and assist with wellness clinics.
- Conduct weekly, monthly, and quarterly audits.
- Manage scheduling of clinical staff.
- Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy.
- Maintain confidentiality of all resident care in accordance with HIPAA guidelines.
- Complete on-going training as assigned.
- Complete other duties as assigned.
- High-School diploma or equivalent.
- Certified Nursing Assistant preferred.
- Must have First Aid & CPR Certification upon hire
- Must have a Food Handlers Card.
- Must have at least 1 yr. of strong experience as a Caregiver or Medication Aide in an Assisted Living Community.

Resident Care Coordinator

Location: Avamere at Bethany, 16030 NW Avamere Court, Portland, OR 97229The primary responsibility of this position is to assist in the management of clinical staff and proper documentation that supports resident care.
