- Develop and implement individualized care plans for residents based on their specific needs and preferences.
- Coordinate with healthcare professionals, families, and support staff to ensure seamless delivery of care services.
- Conduct regular assessments of resident health and well-being, making adjustments to care plans as necessary.
- Facilitate communication between residents, families, and staff to address concerns and enhance the care experience.
- Maintain accurate documentation of care services and resident progress in compliance with regulatory standards.
About the Role:
The Resident Care Coordinator plays a pivotal role in ensuring the highest quality of care for residents at Oaks at Pooler. This position involves coordinating and overseeing the daily operations of resident care services, ensuring that each resident's needs are met with compassion and professionalism. The coordinator will work closely with healthcare staff, families, and residents to develop and implement personalized care plans that promote well-being and enhance the quality of life. Additionally, the role requires regular assessment of care services and making necessary adjustments to improve outcomes. Ultimately, the Resident Care Coordinator is dedicated to fostering a supportive and nurturing environment for all residents.
