What to Expect (Job Responsibilities):
- Provide Care Management services to identified patients
- Conduct assessments and develop care plans
- Coordinate with community organizations on behalf of patients
- Assist with transition planning and crisis management
- Work as part of a multidisciplinary team
What is Required (Qualifications):
- Bachelor's Degree or equivalent education
- Experience in community-based case management
- Strong assessment skills and ability to triage services
- Knowledge of community resources and social service delivery system
- Excellent communication and advocacy skills
How to Stand Out (Preferred Qualifications):
- 2+ years of experience in community-based case management
- Experience in motivational interviewing
