- Duties all duties are under the direct supervision of a registered nurse/practitioner
- Provide holistic evidence based / best practice care to patients in accordance
- with National and Organisational approved policies/procedures and individual care plans.
- Utilise nursing skills to assess, plan and implement and deliver care to patients and review care to patients appropriately.
- Ensure planned nursing care is implemented and evaluated; take appropriate action in the light of evaluation.
- Undertake comprehensive risk assessments of all situations associated with the care of patients in order to ensure nurses and carers safety. This may include assessment and provision of equipment ensuring it is used safely.
- Monitor and maintain the health, safety and security of self and others.
- Play an active role in multi-disciplinary reviews, reporting on the nursing assessment of the patients progress and participating further in Multi-disciplinary Care Planning.
- Be involved in referrals, discharge planning, and case conferences to support or manage appropriate care provision for patients.
- Maintain accurate records, which are confidential, up to date, legible and that all care given must be documented. These records may be paper or IT based system and must be maintained as specified in the Organisation Policies, NMC guidelines and Government directives.
- To function as a member of a multidisciplinary team taking appropriate responsibility for the implementation of decisions made by the team.
- Provide education and continuing support to patients, family and carers.
- Undertake health promotion as requested to all age groups and contribute to planned health promotion activities.
- Be aware of organisational, professional and national developments within primary care and work within these guidelines e.g., NSF/ NICE/ NMC.
- Be aware of and abide by organisations approved Policies, Patient Group Directives, Standards and Quality Assurance Initiatives.
- The returnee is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations/ Chief executive office).
- Be conversant with the vulnerable adult and child protection procedures and policies of Livewell Southwest.
- Always maintain confidentiality in respect to the patient.
- Be responsible for organising own time management daily in line with caseload demands balancing patient needs with the administrative aspect of the role.
- Initiate and participate in Clinical Supervision.
- To liaise effectively with other areas and services. To develop and maintain relationships with providers statutory and non-statutory.
- Act as a professional role model demonstrating clinical skills and high standards of practice and professional conduct.
- Participate in clinical governance and quality improvement initiatives, e.g., clinical audit or clinical policy development.
- The returnee will be required to use a computer, being responsible for timely recording patient activities for IT data collection, dealing with e-mail queries, stock and equipment ordering.
- To preserve confidentiality and comply with of the Data Protection Act, Access to Health Records and Consent for Treatment.
- Have a flexible approach to the working day in order to meet the needs of the patient/service.
- Report to line manager any untoward incidents e.g., complaints, accidents, defective equipment and take appropriate action to ensure safety of patients, relatives, self and colleagues and follow up with appropriate documentation.

- Develop and maintain effective and appropriate communication networks with other members of the Primary Health Care Team and other agencies.
- Have good communication skills to effectively communicate with patients, their relatives and carers about sensitive and accurate information about their condition.

Job summary
Are you a former Adult/General nurse missing the challenges and rewards of nursing? Would you like to return to practice and re-join the NMC register? As an NHS provider, Livewell are looking to recruit committed, compassionate & dynamic former Registered Nurses within our Plymouth District Nursing Teams.
Successful candidates will be supported to complete a Return to Practice programme at Plymouth University. The course is funded, and you will be employed by Livewell Southwest as a Band 3 Healthcare Assistant £24,336 pa pro-rata for the duration of your training. It is recommended that due to the nature of the programme, employed hours are a minimum of 30 hours per week.
On the successful completion of the course, and receipt of NMC PIN, you will then work as a band 5 registered nurse within your allocated team(£28,407 pa, pro rata).
You will have study leave to attend university as required, and you will be supported in practice to achieve your practice learning outcomes. During the course, you will be expected to complete a minimum of 450 clinical hours within your allocated team and commit to attending 8 study days. Main duties of the job
As a return to practice nurse you will be supported & supervised with supernumerary status to work within your scope of practice to undertake clinical duties, manage patient care using evidence based principles, supervise & support junior staff.
The role of the Community Registered Nurse is vital in terms of coordinating peoples care through holistic evidence-based best practice in accordance with National and Organisational approved polices/procedures.
You will be supported to be an autonomous practitioner who is responsible for assessing, triaging, planning, implementation & evaluation of care, and manage sensitive issues.
As a returnee, you will be able to restore competence and have excellent organisational & interpersonal skills. Be an enthusiastic & motivated individual, who thrives on innovation an change, putting those who use our services & their families at the heart of the care provided. Have sound knowledge & judgement providing evidence-based wound management & pressure ulcer prevention, continence management, palliative and end of life care.
