      What to Expect (Job Responsibilities):
      - Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
      - Develop and implement person-centered care plans to address medical, behavioral, and socioeconomic needs
      - Partner and collaborate with internal care team, providers, and community resources to implement care plan
      - Provide education and coaching to support member self-management of care needs and lifestyle changes
      - Support proactive discharge planning and manage/coordinate Care Transition following ER visit or admission

      What is Required (Qualifications):
      - Current, unrestricted independent licensure as a Registered Nurse
      - 3+ years of clinical experience
      - 2+ years of relevant healthcare experience
      - 1+ years of experience with MS Office, including Word, Excel, and Outlook
      - Reliable transportation and the ability to travel within assigned territory

      How to Stand Out (Preferred Qualifications):
      - Master's degree or higher in a clinical field
      - Commission for Case Manager (CCM) certification
      - 3+ years of clinical experience as a Registered Nurse
      - 1+ years of community case management experience
      - Experience working in team-based care
