      The Cardiovascular Case Manager is responsible for planning, coordinating, negotiating and initiating the efficient movement of the patient through the continuum of care, based on medical needs, functional ability and available resources. This position requires population related competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires.       Qualifications

      • Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
      • RN experience should include 5 years clinical experience in an acute care environment OR 3 years' experience with a bachelor's degree in nursing
      • Cardiology experience strongly preferred
      • Must be able to communicate effectively in English

      Responsibilities

      The CV Case Manager responsibilities include, but are not limited to, the following:

      • Assesses the patient continuously for medical necessity, appropriate level of care, financial situation and discharge barriers
      • Responsible for completing physical assessments upon admission
      • Initiates discharge planning and will collaboratively work with the interdisciplinary team to identify and coordinate needed services and resources
      • Provides education to patient's family and staff as required
      • Audits data abstraction for accuracy and provides concurrent feedback to physicians regarding quality measures

      The CV Case Manager reports to the Lead CV Case Manager.

      This position may have additional or varied physical demand and/or respiratory fit test requirements.
