  Position Summary

  The Care Manager will assess each client and family situation by evaluating the client’s level of functioning, health, emotional state, finances, and their legal needs in an effort to enhance their overall quality of life.

  Minimum Qualifications:

  • Master’s degree in a related area preferred.

  • RN License in the state of CA.

  • 5+ years of experience in home care and/or local human service and medical field and resources regularly utilized by older adults (i.e. Medicare, Medicaid, etc.).

  • Excellent communication, organizational, and time management skills.

  • Personal integrity and ethical standards.

  • Computer proficiency in Microsoft Word, Excel, and Outlook.

  Base Pay: $120-140k/annual
