Assess for real time patient clinical, functional and behavioral health status to ensure rapid follow up, allocation of services and episodic management to mitigate avoidable emergent care or acute care hospitalization.
Work closely with the scheduling, authorization & billing team to align resource utilization with authorized visits and ensure documentation integrity to justify medical necessity and avoid unbillable services.
Contribute to the development of new and / or revised work processes, policies and procedures relating to the ASHN Care Management Program.
Support field nurses with physician communication.
Care coordination oversight using Care management technology platform, including referrals to Social Worker, Patient Care Navigators, Health Coaches and/or Federal Qualified Health Centers.
Seek opportunities to contain cost.
Monthly on-site IDT meetings and Agency leadership collaboration.
Oversee the agency care management program with direct accountability & authority for resource allocation and high quality, efficient, cost-effective care delivery.
Demonstrates exceptional collaboration skill and high EQ with all internal and external interactions.
Self-starter and innovative problem solver.
Knowledge of Medicare, Medicaid and all State/Federal guidelines for compliance of patient clinical care.
Strong clinical acumen to thoroughly understand the patient’s health and functional status and foster effective communication with physician office teams.
Able to create positive impressions and communicate effectively with a variety of people and personalities
Is an active listener and demonstrates ability to engage care team in the patient plan of care.
Must project a professional image during virtual communication.
Ability to set up work systems and engage in flexible problem-solving behavior
Observant and detail oriented
Proficiency in Microsoft Excel, Access, Word, and Outlook and the ability to learn new systems.
Registered Nurse required, in state of employment
Two years home care field experience preferred
Minimum of one-year Clinical Review experience preferred
Two years case management and/or utilization management experience preferred
Minimum of one-year EMT preferred
Proficient in HCHB
Capable of all physical demands

You’ll be responsible for the development, oversight and continuity of the patient plan of care, serving as the liaison for collaboration and communication between the field staff and the patients following physician. You will coordinate the interdisciplinary team meetings to ensure tight collaboration and support to the care team as the plan of care is developed and delivered.
