What to Expect (Job Responsibilities):
- Coordinate care for patients with complex chronic illnesses, including care transitions and establishing treatment plans
- Identify and address social determinants of health for patients
- Assess patients for unreported medical conditions or changes
- Work autonomously and collaborate with the healthcare team to facilitate quality, cost-effective care
- Conduct in-person visits at dialysis clinics and nephrology practices within an assigned geography

What is Required (Qualifications):
- Passion for delivering care to patients with complex, chronic illnesses
- Current RN License, BSN preferred
- Minimum of 5 years RN experience with healthcare performance coaching
- Current CPR or BLS certification
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook

How to Stand Out (Preferred Qualifications):
- 2+ years of Case Management or Chronic Care Management
- 1+ years of utilization management experience
- Spanish-speaking preferred
