Banner Home Health is a four-star home care agency with a track record for quality and excellence and is the largest skilled home care agency in Phoenix. Our structure is set up to provide Home Health Nurses multiple avenues for support that aren't available at other agencies – a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in office team of QI specialists, telehealth resources, clinical manager and in office triage support. This structure is designed to assist field clinicians in managing their caseload appropriately and not feeling “alone” in the field! As the RN Clinical Resource Lead, you will assist with patient and clinician needs providing mentorship and clinical support to the field team. Resource Leads also work closely with Clinical Service Managers and office teams to help with staffing and assist with patient complaints.

This position will be scheduled Monday - Friday, on-call rotation every 10 weeks!

The Banner Hospice Team can provide comfort to the patient and family by alleviating symptoms, by providing meaningful help and by creating an environment to develop lasting final memories.POSITION SUMMARY
This position uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost effective patient care. Uses influence to improve patient outcomes and nursing care through individual mentoring and system changes that empower nurses to implement evidence-based care practices. Facilitates assessment and indentification of ongoing learning needs of staff. Ensures seamless orientation of new and/or transferring clinical staff including support and development of preceptors. Skills and competencies include system thinking, risk analysis, techniques in quality improvement at the micro and mesosystem level, use of evidence-based practices, and the ability to laterally integrate care over the healthcare continuum.

2. Uses appropriate teaching/learning principles and strategies as well as current information, materials and technologies to facilitate the learning of health care professionals.
3. Facilitates assessment and identification of ongoing learning needs and evaluates learning effectiveness.
4. Creates processes for sharing information and decision-making across the care settings and roles.
5. Leads the integration of innovation and evidence based findings into practice. Creates an environment supportive of innovative solutions. Assumes a leadership role, in partnership with physicians and facility leaders, in managing the quality of services provided across the care delivery continuum.
6. Coordinates quality initiatives and process improvements.
7. Responsible for selection, orientation, on-boarding, and retention of assigned staff. Demonstrates leadership through coaching, performance evaluations, corrective actions, and development opportunities to create a culture of learning.
8. Contributes to evidence-based practice through generating, translating, and/or implementing research. Discovers, examines, and evaluates knowledge and theory.
9. Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient.
MINIMUM QUALIFICATIONS
Must possess a strong knowledge of healthcare as normally obtained through the completion of a master's degree in nursing, healthcare administration, or related field.
RN license in state of practice, two years of clinical expertise required, BLS
Critical thinking skills, clinically competent to care for patients, knowledgeable about disease processes and can identify symptoms of complications. Skilled in psycho-social evaluation of patient and family. Effective in identification of the need for consultation services of Advanced Practices Nurses and other allied health providers. Knowledgeable about pharmacologic therapies and drug interactions. Able to integrate care of multiple disciplines to achieve highest outcomes.
PREFERRED QUALIFICATIONSOne to two years nursing leadership experience is preferred.
