What to Expect (Job Responsibilities):
- Assess and evaluate the needs of complex members
- Develop and evaluate medical management strategies and interventions
- Act as an advocate and liaison for members
- Ensure compliance with all regulations and standards
- Enter and maintain documentation in the Electronic Medical Records System

What is Required (Qualifications):
- Bachelor’s degree
- RN required, with current active RN license
- CCM certification
- 4 – 6 years of clinical experience
- Strong communication skills

How to Stand Out (Preferred Qualifications):
- Experience in case management/care coordination
- Trained in the use of Motivational Interviewing techniques
- Bilingual ability
