- With Administrator, assess applicants for appropriateness for admission to Evergreen
- Update Appendix C resident assessment annually
- Assess individual residents monthly for signs/symptoms of changes to general health statu s , and for smoking status ; assess for proper self-administration of injectable meds and care/use of CPAP.
- Confirm the accuracy of MARs and medications to be administered with medical provider orders
- Quarterly evaluation of CNA/Medication Aides for identified skills
- Pharmacy coordination for medication fulfillment.
- Medication reconciliation and Provider communication with VA, PCP’s and medical specialists
- Schedule is flexible with parameters including availability to check in scheduled monthly med delivery and meet with all clients and CNA/Medication Aides regularly .

Location: Woonsocket, RIRN, Evergreen Assisted Living (Part Time)
Evergreen Assisted Living, in Woonsocket RI, is a 26-bed F2, M1 assisted living residence in Woonsocket, RI, specializing in serving adults with mental illness .Evergreen seeks a compassionate, professional Registered Nurse (Part time) (8-10 hours/week), preferably two 4–5-hour days per week, but negotiable.
RN will work collaboratively with CNA/Med Techs, caseworkers, community providers , the agency Director of Nurses, and the Administrator to care for adults with behavioral health disorders .
