      RN MDS Coordinator – Napa Post Acute
      Location: Napa, CA
      Pay: $110,000–$120,000/year (DOE)
      Status: Full-Time

      Are you an experienced RN with a strong understanding of MDS and care planning? Napa Post Acute is seeking a detail-oriented RN MDS Coordinator to lead our assessment and care planning processes with accuracy, compassion, and clinical excellence.

      What You’ll Do:

      • Coordinate and oversee the MDS assessment process to ensure timely and accurate submissions

      • Develop and update individualized care plans with the interdisciplinary team

      • Ensure compliance with all federal and state regulations, including PDPM guidelines

      • Collaborate with clinical and administrative staff to optimize resident outcomes and reimbursement

      • Provide staff education related to MDS processes and documentation

      What We’re Looking For:

      • Active Registered Nurse (RN) license in California

      • Minimum 1 year of MDS experience in a skilled nursing facility

      • Strong knowledge of PDPM, CMS regulations, and RAI process

      • Excellent organizational, communication, and critical thinking skills

      • Passion for delivering quality care to seniors
