What to Expect (Job Responsibilities):
- Call, track, report, and monitor patients enrolled in the remote patient monitoring program
- Respond to patient results, provide lifestyle coaching, and patient education
- Assist complex patients in the transition of care across multiple settings
- Provide targeted interventions to avoid hospitalization and emergency room visits
- Coordinate a patient care delivery system for a designated group of patients
What is Required (Qualifications):
- Minimum of three years nursing experience
- Bachelor's degree in nursing (BSN) required
- Effective verbal and written communication skills
- Ability to prioritize and make independent clinical judgments
- Experience with basic patient care equipment
- Ability to work collaboratively with an interdisciplinary team
- Effective critical thinking/analytical skills
- Currently licensed as a Registered Nurse in the state of Nebraska or current compact state license
- Basic Life Support (BLS) certification
How to Stand Out (Preferred Qualifications):
- Prior nursing experience in related field
- Master's of Science in Nursing (MSN) degree
- Knowledge of health care financial environment and reimbursement systems
- Experience leading/supervising a team
- Certification in relevant areas such as Advanced Cardiac Life Support (ACLS) or Oncology Nursing Certification (OCN)
