What to Expect (Job Responsibilities):
- Assess and determine the timely acceptance or decline of patient referrals based on established criteria
- Maintain communication with clients, family members, and referral sources to ensure optimal patient satisfaction
- Collaborate with agency staff and sales teams to achieve desired patient outcomes
- Facilitate the collection of all necessary information and documentation for referrals
- Adapt to changing territorial assignments as needed by the employer

What is Required (Qualifications):
- Licensed LVN or PT in the practicing state
- 3-7 years of experience in facility/physician relationships with a strong understanding of facility discharge processes
- Minimum of one year of experience in home care, hospice, or personal care services
- Prior experience with Electronic Medical Records (EMR)
- Familiarity with various healthcare payors, including Medicare, Medicaid, and commercial payers
