What to Expect (Job Responsibilities):
- Timely acceptance or decline of patient referrals based on predetermined criteria
- Establish and maintain communication with clients, family members, and referral sources
- Partner with agency staff and sales to ensure optimal patient satisfaction and outcomes
- Facilitate the collection of all necessary information and documentation to complete referrals
- Adapt to the changing needs of the company by managing assigned territory, which may encompass multiple states

What is Required (Qualifications):
- Licensed LVN or PT in the practicing state
- 3-7 years of experience in facility/physician relationships with a deep understanding of facility discharge processes
- Minimum of one year of experience in home care, hospice, or personal care services
- Prior use of Electronic Medical Records (EMR)
- Experience dealing with various payors in healthcare, including Medicare, Medicaid, and commercial payers
